=== Sniffy the Rat === On 28 December 1989, The Province newspaper in Vancouver, Canada, reported that Gibson intended to crush a rat named Sniffy between two paint canvasses with a 25-kilogram concrete block in downtown Vancouver. On impact, Sniffy would leave an imprint on the canvasses, forming a diptych. Gibson said he had acquired Sniffy from a pet shop which sold living rats as food for snakes and lizards. The performance was planned to happen on 6 January 1990, outside the old central public library on Burrard Street. Opinion about the impending event was publicly broadcast via newsprint, television, and radio. On the morning of 6 January, a group of animal rights activists from the Lifeforce Foundation stole the device Gibson was going to use to crush the rat. Lifeforce's Peter Hamilton said that it was done to protect both the rat and Gibson. Because of this development, Gibson arrived at the corner of Robson and Burrard at 1:00 pm without Sniffy or his art-making device. He told a crowd of over 300 people that he had returned the rat to the pet shop from where he had rented it. He encouraged the crowd to go to the pet shop and rescue Sniffy before it was sold as snake food. He later told CBC that he had full intentions of killing the animal. As he tried to leave the area, Gibson was surrounded by activists. He, along with Susan Milne and Paddy Ryan, were chased up Burrard Street by a mob. The three of them escaped through the Hotel Vancouver. Later that day, Sniffy was purchased from the pet shop by Peter Hamilton of the Lifeforce Foundation.

==== Warning signs ignored ==== From 2005 to 2006, an agent, Jiang Weisuo, from Shaanxi Jinqiao Dairy Company in northern China reportedly publicly discussed his fears about unauthorised substances being added to competitors' milk. His complaints to regulators and dairy makers in 2005 and 2006 never yielded any result; his story was picked up by China Central Television, who ran a report, complete with footage of adulteration in progress, yet the Shaanxi Quality and Technical Supervision Bureau said they failed to find evidence of wrongdoing. The bulletin board of the Administration of Quality Supervision, Inspection and Quarantine (AQSIQ) indicated a rare occurrence of kidney stones in children–all causally traced to Sanlu milk formula–was flagged by at least one member of the public in June 2008 and by a urologist in a paediatric hospital on 24 July 2008. Neither received definitive replies. The paediatrician, who specifically asked the AQSIQ to refer his observations to epidemiologists, was asked to refer his query to the health department. In June, Jiangsu media reported a two-month surge in the number of babies diagnosed with kidney disease; in July, a parent of a sick baby in Hunan questioned Sanlu's powdered milk and complained to the AQSIQ. Gansu Province sent a report to the Ministry of Health on 16 July to alert that one local hospital had identified an increase in the incidence of kidney ailments among babies in the months earlier, and that most victims had consumed Sanlu's baby formula. The health ministry sent investigators to Gansu in early August.
== Causes == AMP deaminase is an enzyme that converts adenosine monophosphate (AMP) to inosine monophosphate (IMP), freeing an ammonia molecule in the process. It is a part of the metabolic process that converts sugar, fat, and protein into cellular energy. In order to use energy, a cell converts one of the above fuels into adenosine triphosphate (ATP) via the mitochondria. Cellular processes, especially muscles, then convert the ATP into adenosine diphosphate (ADP), freeing the energy to do work. During heavy or prolonged mild to moderate activity, other enzymes convert two molecules of ADP into one ATP molecule and one AMP molecule, making more ATP available to supply energy. AMP is normally converted into IMP by myoadenylate deaminase—so myoadenylate deaminase deficiency reduces energy that would be available to the cell through the purine nucleotide cycle. Instead of being converted to IMP, the AMP builds up in the cells of affected individuals, spills into the blood, and is eventually metabolized in the liver. In persons with a defective enzyme, 5'-nucleotidase removes the ribose and phosphorus from AMP, increasing levels of adenosine measured in muscle cells by ~16–25×, after exercise.

=== Gastroesophageal junction === The gastroesophageal junction (also known as the esophagogastric junction) is the junction between the esophagus and the stomach, at the lower end of the esophagus. The pink color of the esophageal mucosa contrasts to the deeper red of the gastric mucosa, and the mucosal transition can be seen as an irregular zig-zag line, which is often called the z-line. Histological examination reveals abrupt transition between the stratified squamous epithelium of the esophagus and the simple columnar epithelium of the stomach. Normally, the cardia of the stomach is immediately distal to the z-line and the z-line coincides with the upper limit of the gastric folds of the cardia; however, when the anatomy of the mucosa is distorted in Barrett's esophagus the true gastroesophageal junction can be identified by the upper limit of the gastric folds rather than the mucosal transition. ==== Peripheral muscle fatigue ==== Peripheral muscle fatigue during physical work is considered an inability for the body to supply sufficient energy or other metabolites to the contracting muscles to meet the increased energy demand. This is the most common case of physical fatigue—affecting a national average of 72% of adults in the work force in 2002. This causes contractile dysfunction that manifests in the eventual reduction or lack of ability of a single muscle or local group of muscles to do work. The insufficiency of energy, i.e. sub-optimal aerobic metabolism, generally results in the accumulation of lactic acid and other acidic anaerobic metabolic by-products in the muscle, causing the stereotypical burning sensation of local muscle fatigue, though recent studies have indicated otherwise, actually finding that lactic acid is a source of energy. The fundamental difference between the peripheral and central theories of muscle fatigue is that the peripheral model of muscle fatigue assumes failure at one or more sites in the chain that initiates muscle contraction. == Overdose == Overdoses of hydroxychloroquine are extremely rare, but extremely toxic. Eight people are known to have overdosed since the drug's introduction in the mid-1950s, of which three have died. Chloroquine has a risk of death in overdose in adults of about 20%, while hydroxychloroquine is estimated to be two or threefold less toxic. Serious signs and symptoms of overdose generally occur within an hour of ingestion. These may include sleepiness, vision changes, seizures, coma, stopping of breathing, and heart problems such as ventricular fibrillation and low blood pressure. Loss of vision may be permanent. Low blood potassium, to levels of 1 to 2 mmol/L, may also occur. Cardiovascular abnormalities such as QRS complex widening and QT interval prolongation may also occur. Treatment recommendations include early mechanical ventilation, heart monitoring, and activated charcoal. Supportive treatment with intravenous fluids and vasopressors may be required with epinephrine being the vasopressor of choice. Stomach pumping may also be used. Sodium bicarbonate and hypertonic saline may be used in cases of severe QRS complex widening. Seizures may be treated with benzodiazepines. Intravenous potassium chloride may be required, however this may result in high blood potassium later in the course of the disease. Dialysis does not appear to be useful.

The Windscale fire brought the need for testing of the land downwind for radioactive material contamination, and this is how it was found. An estimate of 8.8 terabecquerels (240 Ci) of polonium-210 has been made. It has also been suggested that Irène Joliot-Curie's 1956 death from leukaemia was owed to the radiation effects of polonium. She was accidentally exposed in 1946 when a sealed capsule of the element exploded on her laboratory bench. As well, several deaths in Israel during 1957–1969 have been alleged to have resulted from 210Po exposure. A leak was discovered at a Weizmann Institute laboratory in 1957. Traces of 210Po were found on the hands of Professor Dror Sadeh, a physicist who researched radioactive materials. Medical tests indicated no harm, but the tests did not include bone marrow. Sadeh, one of his students, and two colleagues died from various cancers over the subsequent few years. The issue was investigated secretly, but there was never any formal admission of a connection between the leak and the deaths. The Church Rock uranium mill spill 16 July 1979 is reported to have released polonium-210. The report states animals had higher concentrations of lead-210, polonium-210 and radium-226 than the tissues from control animals.
